Ethicon is a Johnson & Johnson company that manufactures a number of types of surgical mesh made of a variety of different materials. Physiomesh is a partially absorbable mesh designed for hernia repair made of polypropylene and coated with polyglecaprone. It is available in a number of sizes and features a hexagonal shape. Its transparency provides for good visibility and is easily handled during surgery. In order to reduce the risk of inflammation and adhesions, Physiomesh includes layers of absorbable Monocryl. Physiomesh was approved in 2010. Prior to Physiomesh, Ethicon created Proceed in 2003. Proceed is made of polypropylene and is coated with bioabsorbable oxidized regenerated cellulose that is intended to separate the mesh from organs and tissue.

Ethicon has not been able to avoid controversy with its products, like many manufacturers of surgical mesh. Ethicon recalled a number of its Proceed Mesh units in 2005, 2010 and 2014 because of a problem with the loss of laminate coating on the mesh that would allow the mesh to come in direct contact with organs and increase the chance of adhesions and fistulas. In May 2016, an Urgent Field Safety Notice was issued for Ethicon’s Physiomesh Composite Mesh and Laparoscopic Hernia kits and Ethicon withdrew them from the market, resisting the recall label in the United States. The withdrawal was based on higher rates of recurrence and revision operations. Ethicon is going to face its first lawsuit in 2018. The plaintiff in the case was implanted with Physiomesh in 2013 and developed severe complications two years later. He experienced severe pain, fever, and nausea. Once being treated, he learned that abscesses and an intestinal fistula had formed, requiring a revision surgery.

Many consumers have filed lawsuits against Ethicon based on complications faced after Proceed and Physiomesh were implanted during hernia repair. In June 2017, an MDL was established for the cases against Ethicon in the Northern District of Georgia. These suits are ongoing and Plaintiffs continue to join them. The lawsuits claim that the polypropylene used to make Physiomesh has caused negative reactions with tissues and organs in such a way that it impacts the patient’s health adversely. The coating on the mesh intended to guard against direct contact between mesh and abdominal organs is also seemingly not performing as expected.

The coating may prevent adhesions and scar tissue, but it may also be preventing the mesh from absorbing correctly into the body, increasing the risk of mesh migration. Other studies showed that Physiomesh did not perform well when compared with other devices, including:  seromas, scar tissue, inflammation and bleeding. Anyone who has been implanted with surgical mesh and experiences complications should get immediate medical attention to avoid even greater consequences.
